During this time of uncertainty, we all have a lot of questions concerning athlete participation. It is my goal in this post to provide answers for you in regards of athlete participation requirements.
- The IHSAA is permitting athletes who have received a 2019-20 physical to play sports during the 2020-21 school year. However, to ensure the total safety of every student-athlete who participates in athletics, we strongly recommend that all athletes get a new physical for this year from their primary doctor, pediatrician, or IHSAA approved medical staff. In these times we encourage athletes to please get a new physical examination because they are more important than merely clearing athletes to play sports. I have included a letter written by area physicians that support getting a new physical at the bottom. All athletes who participate in athletics will need a physical on file or an updated Final Forms account prior to the first practice.
- All parents, guardians or an emancipated student must complete the 2020-2021 IHSAA Health History Update Questionnaire and Consent & Release Certificate. This certificate can be obtained at the bottom and then turned into the Athletic Department. Also, Final Forms is including this document into every athlete’s account so it can be completed online. This certificate must be completed prior to the first workout which begin the week of July 6th. Athletes who have not this completed this form will not be permitted to attend or participate in conditioning or summer workouts until completing this document.
I hope this helps.
